Welcome to our website! Hi, my name is Tyler Hoemberg and I serve as Administrator/CEO of Bridges Care Community. I completed a Bachelor’s Degree in Healthcare Management from Concordia College-Moorhead in 2012 and am currently licensed as a Nursing Home Administrator in the state of Minnesota. Under my license, I oversee the functions of Bridges Care Community and the services we provide. My most imperative duties are to bring our Mission and Core Values of Hospitality, Stewardship, Justice, and Respect to life at Bridges and to develop our vision and strategic plan in response to the needs of our community. I moved to Ada from Staples in November 2012. I enjoy the small town feel the community provides and the growth it is experiencing. My favorite hobbies outside of work include sporting activities, ice fishing, hunting of all kinds, and being at the lakes during the summer months. My name is Cindy Scherfenberg and I am the Director of Nursing here at Bridges Care Community. I am responsible for leading, planning, organizing, staffing, directing, analyzing and evaluating the nursing services of Bridges Care Community. I also supervise all nursing staff to ensure that our nursing services are the best possible. I have a Bachelors Degree in Nursing from Moorhead State. I have worked at Bridges for 17 years. I started in the clinic while attended nursing school and then moved into the hospital setting as a nurse. In 2001, I started working in the Care Community as a RN doing assessments and eventually learning to complete the MDS. I moved into the role of Director of Nursing in April of 2006. I am married and have 3 daughters; Elle, Brooke and Lacey. While not at work, I enjoy spending time with my family, going to the lake and taking vacations with my family. I truly enjoy the residents and the staff at Bridges. The residents are just like my own family because of the time we spend caring for them. Bridges also has wonderful staff. If I had a loved one that needed long term care this is where I would bring them. The staff are compassionate and caring. They try their best everyday to make Bridges their home. Greetings from Bridges Care Community! My name is Erin Aanenson and I am the energetic Wellness Director! I plan and implement all leisure activities that take place daily. I am also the volunteer coordinator who maintains, recruits, and looks for volunteers to serve our residents and help out our staff. I have had the honor of being employed by Bridges Care Community for three years! I received a Bachelor of Science degree in Communication with an emphasis in Marketing from the University of Minnesota, Crookston and hold my National Certification as a Wellness Director. The number one thing I love about my job is the residents. They have such a unique past; their own story that is truly shown in their personalities. The staff at Bridges are wonderful to work with and we have our own little family here. We are so blessed to have a beautiful facility which continues to have innovative ideas implemented throughout, such as ceiling track lifts. I grew up in Ada and before coming to Bridges Care Community I lived in Tallahassee, Florida and worked for Florida State University as an admissions officer. In my spare time you will find me enjoying my daughters Chloe and Cora, spending time with my husband, scrap booking, exercising or just simply having fun somewhere!

My name is Christina Jacobson and I’m the Business Office Manager for Bridges Care Community and Bridges Supportive Services. As the Business Office Manager I’m responsible for many different items: I’m head of human resources, responsible for accounts payable, accounts receivable, timekeeping, payroll, billing, resident trust accounting and various other business office functions. I also lead the recruitment and retention committee within the facility which is made up of various employees working towards the common goal of making Bridges an excellent place to work. As an employee of Bridges Care Community, I have been here since 2006 and absolutely love the people. Everyone from my coworkers to the family members of those living here are wonderful to be around. Also, the building itself is beautiful! I love that the facility seeks growth opportunities and advancements in modern technology. Being a smaller, rural facility, these items put us above the rest and show our ambition to be the best we can be. I grew up in Halstad, MN and didn't move too far from home. I am married and together we have two sons, Joe and Calvin, who keep us VERY busy! In my spare time I enjoy being outdoors; whether its 85 degrees or 10 below. Traveling is another one of my favorite things to do, be it a road trip to Yellowstone or a flight to New York City. Traveling to various baseball stadiums and finding the best of the best places to eat make for a great vacation. My name is Wendy Bennefeld, Social Worker. I have been employed with Bridges Care Community for five years. I currently reside in rural Gary, MN with my husband and daughter. I was originally employed as the Health Unit Coordinator and after attending Minnesota State University in Moorhead I have moved into the social service area at Bridges Care Community. I recently completed a Bachelors Degree in SW and Gerontology which allows me to specialize in the unique needs and challenges that face the geriatric population. Entering a Skilled Nursing Facility can be a difficult decision for individuals and their families. I strive to ease that anxiety and help individuals and families find comfort in their decision to allow Bridges Care Community to provide the services needed while maintaining their dignity and independence. Attending to the psycho-social wellbeing of each resident through visits, formal assessments and getting to personally know each resident is the most important role I have at Bridges. My favorite thing about Bridges is being a part of a Catholic organization that recognizes and attends to the individual needs of each person who chooses to reside here. My name is Elizabeth Thompson, the Supportive Services Director. I am excited to be the newest member of the leadership team here at Bridges Care Community! I direct in all the nutritional aspects of our residents care by over seeing Culinary Services, as well as Housekeeping and Maintenance Departments. I received an Associates Degree in Travel Business Management and Accounting from Interstate Business College and have 15+ years in the Customer Service industry. I really enjoy getting to spend time with the residents and working on making their lives at the Care Community exceptional. It is such a good feeling to be working with the staff who continually strive to make everyone feel like a part of the Bridges family. I am married and have 2 sons; Braden and Zander.
