Prince of Peace Care Center of Ellendale and St. Rose Care Center of LaMoure are excited to announce that they have received a matching grant from Benedictine Health System for a Parish Nurse in each of their respective counties. The grant was applied for with support of Dickey/LaMoure Ministerial Associations, Dickey/LaMoure Social Services, the schools of both counties and Dickey/LaMoure Public Health to provide a program for those individuals of need.


Purpose: Is to be a Single Point of Entry while providing an integration of spiritual and health services


Targeted Client Group: Children, disabled and elderly of all ages that have psychological, physical and spiritual needs


Goal: To promote healthy living while focusing on providing choices to clients that will enable them to live as independently as possible


Process: Using a Single Point of Entry (SPOE) model, the nurse will receive referrals from the local churches and agencies. A meeting with the client will follow to perform an initial assessment, discuss what the clients' needs are and identify any existing gaps. The Parish Nurse will offer spiritual guidance or support through their clergy of choice and arrange referrals to other collaborating agencies.

 
 

Continuation: Future continuation as the program develops is based on parish funds, client donations, operating funds from Prince of Peace Care Center and St. Rose Care Center facilities, grants, fund development and memorial funds.
