Awards Banquet: An annual employee banquet is held each year to honor employees for their service. Annual Physical: After completion of 1 year of employment, Saint Anne’s will pay up to $25.00 towards an annual physical. Proof of the physical (including a health care provider’s signature) must be provided. Employee Assistance Program (EAP): An Employee Assistance Program is available to assist employees and their immediate families in finding help for emotional, mental, family, health drug/alcohol, legal and financial problems. This service is free and confidential. Extended Illness Bank (EIB): Employees accrue hours in an extended illness bank to compensate for absences over 3 consecutive days resulting from illness or injury. Finders Fee: Employees will receive a $100.00 finders fee if someone they refer for employment is hired and completes their probationary period. Flexible Spending Accounts (FSA): Employees will be eligible for flexible spending accounts if they are guaranteed 20 or more hours per week. The health care reimbursement account provides an opportunity to set aside a portion of annual pay on a pre-tax basis to pay for eligible health care expenses. The dependent care reimbursement account provides an opportunity to set aside a portion of annual pay on a pre-tax basis to pay for qualified child care expenses. Funeral Pay: Employee shall be entitled up to 3 consecutive paid days off from scheduled work to attend the funeral of the employee’s spouse, child, parent, sibling, or parent-in-law. One day will be paid upon the death of a grandparent or grandchild to attend the funeral, if the employee is scheduled to work that day. Holidays: After the probationary period, employees regularly scheduled less than forty-eight (48) hours per pay period who work on a holiday shall be paid at the rate of 1½ his/her regular straight-time rate of pay for all hours worked on the holiday. Holiday Meals: Saint Anne of Winona will provide a dinner meal to employees working on a holiday. Insurance: Employees will be eligible for health insurance if they are guaranteed 20 or more hours per week. Dental, term life and accidental death/dismemberment insurance are available for employees guaranteed 24 or more hours per week. Jury Duty: Saint Anne of Winona will pay your regular rate of pay consistent with your regularly scheduled work hours during jury duty. Any payment received from the court, except mileage, must be submitted to the Business Office to receive payment. This benefit shall be limited to 160 hours per year. Leave Of Absence: Unpaid leaves of absences are available for illness, injury, education, and personal reasons. Nursing Assistant Tuition Reimbursement: Upon certification and 90 days of employment at Saint Anne of Winona, total tuition and test fees will be reimbursed to Nursing Assistants. Employees must present original receipts of payment to Human Resources to receive payment. Paid Time Off (PTO): Paid time off is accrued and available to employees regularly scheduled 48 or more hours per pay period after the probationary period. PTO will accrue on all hours paid, up to 80 hours per pay period. PTO may be carried over from year to year, subject to limits. Retirement (401(k) Plan): A 401(k) plan is available to all employees. Employees may start contributing upon employment. After 1 year of service, 21 years of age and 1,000 hours worked, eligible employees receive a 1.5% employer contribution and 1.5% employer match contribution(if contributing at least 2% of their pay).
