The Transitional Care Unit at the Benedictine Health Center at Innsbruck is dedicated to providing skilled medical care for adults who plan on returning home.  We offer qualified, specially trained staff and professional therapy services in an atmosphere of service and hospitality.  Our daily program provides skilled nursing care and physical, occupational, and speech therapy as ordered by your physician.   Spiritual support is woven throughout our program.

 

 

Our Program

We begin planning for your discharge on the day you arrive. The daily schedule is busy with therapy appointments and restorative care. To assist you in returning home soon, we begin immediately on admission to:

  • Coordinate your rehabilitative and restorative care to maintain or improve your functioning.
  • Assist you in the recovery from an acute illness or injury.
  • Support you as you adapt to possibly irreversible change in physical and functional status.
  • Maximize your level of independent functioning through assistance in relearning the functions of activities of daily living (ADL).
  • Facilitate community re-entry and lifestyle management.

Our team of specially trained staff includes nurses, physical therapists, occupational therapists, speech therapists, dieticians, and social workers who will provide skilled care and therapy focused on getting you home as soon as possible. Physicians, nurse practitioners, and physician assistants are on site as necessary.
