Social Services St. Eligius Health Center offers the following the following services: Offering support and advocacy for resident and family Assisting with discharge planning-arranging needs such as home health care for the transition home, ordering medical equipment and assistance with placement. Providing community resource guidance Supplying information and education regarding Power of Attorney for Health Care Decisions, Power of Attorney for Financial Decisions, Health Care Directives and Legal Guardianship Assisting with referrals to the individuals permanent residing county for financial and program assistance matters.
